The Immunization Deadline for students enrolled in Fall 2025 is September 16, 2025

North Carolina law requires individuals attending a college or university, whether public, private or religious, to receive certain immunizations. The registrar of the college or university is responsible for ensuring that all new (undergraduate and graduate) students enrolling in college at any time during the year have received the required immunizations.

The statute applies to all students except students residing off-campus and registering for any combination of:

  • Off-campus courses
  • Evening courses (classes beginning after 5 p.m.)
  • Weekend courses
  • No more than four days of credit hours in on-campus courses

Per North Carolina state law, students will be withdrawn from the university 30 days after classes begin if immunization requirements have not been met.

Required Immunizations

Immunization Information

Students entering college or university for the first time on or after July 1, 2008, must have had at least three doses of tetanus/diphtheria toxoid; one of which must be a Tdap booster (tetanus, diphtheria, and pertussis).

The Tdap booster must have been given between the ages of 10 and 12 or when entering the 7th grade.

An individual attending school who has attained their 18th birthday is not required to receive the polio vaccine.

Both doses must have been given on or after the first birthday.

Antibody testing confirming immunity to measles will be accepted instead of the vaccine.

Antibody testing confirming immunity to mumps will be accepted instead of the vaccine.

Students who entered college or university before July 1, 2008, are not required to receive the second dose of the mumps vaccine.

Antibody testing confirming immunity to rubella will be accepted instead of the vaccine.

A two-dose series of Heplisav-B is acceptable (this vaccine is only available in the United States).

Antibody testing for Hepatitis B will NOT be accepted. Students must show documentation of a completed Hepatitis B vaccine series.

Hepatitis B vaccine is not required if a student was born before July 1, 1994.

Documentation from a medical provider verifying varicella disease is accepted instead of vaccine dates.

Antibody testing confirming immunity to varicella will be accepted in place of the vaccine.

If a student was born before April 1, 2001, a vaccine for varicella or proof of immunity is not required.

Must have been given on or after the 16th birthday.

Vaccine brands that satisfy this requirement are: Menactra, Menveo, MenQuadfi, and Penbraya.

Meningococcal B vaccines (Bexsero & Trumenba) will NOT meet this requirement.

Meningococcal is not required if a student was born before January 1, 2003.

Immunization Exemption

Exemption Requests can be submitted through the Patient Portal.

Medical exemptions for required immunizations will only be granted upon submission of either a Medical Exemption Statement (DHHS 3987) or a Physician's Request for Medical Exemption (DHHS 3995). Both of these exemptions require a physician's signature in North Carolina.

The Medical Exemption form(s) must be submitted with the North Carolina Required Immunization and Health History Form.

Religious exemptions for required immunizations will only be granted upon submission of the "Request for Exemption from Immunization Requirement: Religious Reasons" Form.
